The Railway Minister said the reason behind the train collision will be revealed soon. However, some people think that an audit report from last year showed that there were many problems with rail safety. A senior Railways official also warned about a faulty signaling system in February, which is now being investigated as the cause of the accident.

India’s top auditing body looked into derailments in the Railways and found that there were serious problems with inspections, inquiry reports, funding, track renewal, and safety staffing. The report said that there were shortfalls in inspections and failures in the Track Management System.
